Something along the lines of the QGIS create_changelog.sh is probably
sufficient:

 https://github.com/qgis/QGIS/blob/master/scripts/create_changelog.sh

It has the date and author in reverse order to the GNU convention, but
matching the `git log` order.

git2cl is another option and packaged in Debian/Ubuntu & Fedora:
