AJAX Layouts Sometimes Don't fully load

Since installing 1.0, I have noticed that web layouts sometimes don't fully
load.  This is a layout embedded in an HTML frameset.  In these cases, the
surround fully loads, and the legend displays all of the group and layer
names with question marks where any legend images would go next to the layer
names.  The map does not load and the map frame remains blank.  There are no
error messages in the surround or recorded in the server error log.  Hitting
the refresh button to reload the page will usually work to get the layout to
fully load.  There does not appear to be any consistency to when this
happens.  Perhaps it's a session issue?
