By chance are you using DWF Viewer?  Base layers only have their performance benefit when using the AJAX viewer.
 
Also as Traian mentioned, keep the number of base layer groups to a minimum - 1 or 2.  It doesn't matter how many base layers are in a group - it takes time to render the tiles initially, but once rendered the time to access is independent of the number of base layers.
 
Also note that tiles are rendered / cached on demand.  You might have been using your tiled map for some time, but if you explore a new area of the map where there still aren't any tiles then it will initially be slow.  I'm sure someone will one day post a little utility which pregenerates all tiles for a given extent / range of scales, and then this wouldn't be a problem.
