You talk about adding images at run-time...why?  

If you have a custom image for every building, I suspect that with a sizable map you will see some performance problems as the server tries to render the map.  If you are really wanting to show a picture of an individual building, you could use a common symbol for the map then as you hover over the building show the image in a popup window.  If you are simply looking as different images for different building classes, you could theme the layers using the symbol library.
