[Mapserver-dev] Changing Colors in Map !!

Sean's right, those are the main causes, although labels without
antialiasing are no big deal. The old versions of MapServer had a
palette object that was built as the mapfile was parsed. It did in fact
reserve an excess number of colors, but you never heard anyone complain
about missing colors so it was definitely better than what's there
now...

Stephen, your problem is caused by rasters, but i have also seen it
caused by labelling.  Truetype labels can eat up a lot of color space
even without antialiasing.
