[mapserver-dev] Standardized header: Code from other source

It seems that you propose two changes:
1- Move the note about the provenance of the original code (website and
source download URL) into the "Notes" section of the header.
- I'm fine with that.
2- Remove the second copyright line in the main header (the one that
said "Copyright (c) 1997-1998 Sun Microsystems, Inc.") and keep only the
UMN copyright.
- This may be fine since there is still a ref to that Sun copyright line
below the std mapserver license, but I had included the second copyright
in the main header to make it clear that some code in the file was
copyrighted by Sun and it was not all UMN. I'm not a licensing
specialist, so I'm not sure which way is best.
