[mapserver-dev] Doc guidelines reminder ("MapServer")

I just noticed that our docs contain both "Mapserver" and "MapServer"
references.  I want to remind everyone to use "MapServer", and to review
those specific points in the Documentation Guide
(http://www.mapserver.org/development/documentation.html#general-guidelines).

I've fixed over 60 references now in git-master.
