Um, yes it can be done pretty easily. You need freetype installed and then need to use the "ANGLE AUTO" parameter in your label object. The test suite site (i.e. arachnid.dnr.state.mn.us/ms_test) has several auto rotation examples.

Note that the quality of the labeling is heavily dependent on the data. Garbage in, garbage out. Extra nodes can drastically affect placement. Interfaces I've seen using TIGER and GDT data look pretty darn good at the street level.
