I forgot to ask which version of Mapserver you are using? I think the 3.5
version did away with QUERY so instead you put the template in a class
definition something like
LAYER
NAME "lakes"
DATA lakespy2
STATUS ON
TYPE POLYGON
CLASS
NAME "lakes"
TEMPLATE ejem3.html
END
END

I don't think dbf field names are allowed to have spaces in them. If
somehow you got your field name to have a space in it, I would change it
from LAKE NAME to LAKE_NAME.
