Your raster image needs to be georeferenced, by either being a GeoTIFF
image or by having an ESRI-format "World File" associated with it.  That
information will position the raster layer in projected coordinate
space, just as other map layers are.
 
The "extents" of the picture define the portion of the world you're
looking at on a given map; that's a different set of values.  MapServer
uses the requested extents of the output picture to determine which
parts of each of the map layers (if any) need to be drawn in order to
produce that image.

How do you add a raster image layer to your map file? I know you can
pick the type raster, but how do you set the extents of the picture.
