Mapserver 4.2 does support WMS 1.1.1. ESRI ArcExplorer does however work
with 1.1.0.

Good question. I don't think this will be possible out-of-the-box, but I
could be wrong. I only know of an option to supply a user/password to a
WMS server which is protected by Basic Authentication. See e.g.:
