Ka-map problems: problem attached as document

Hi,

I have attached a word document that briefly explains
the problem I'm having with ka-map within Mapserver. I
have looked at everything I could find but I'm sure
it's something simple I have overlooked. 

The attachment is a Word document (zipped) which
contains the problem, the system I'm running and what
solutions I have tried.

I trust this is acceptable to all?
