Is fast-cgi running within mapserver when setup in the following way: MS4W setup with latest FWTools bin content copied into MS4W apache\cgi-bin.

I can't speak for your FWtools-to-MS4W port, but note that MS4W 2.2.2
contains MapServer compiled with FastCGI support (notes here
http://www.maptools.org/ms4w/index.phtml?page=README_INSTALL.html#d-fastcgi),
and you'll need to compile the necessary Apache module (as noted here
http://mapserver.gis.umn.edu/docs/howto/fastcgi/#ms4w-users).

Or i suppose you could use an older Apache version that has a mod_fastcgi
module already available.

(but this is the extent of my knowledge, as stated in one of the docs the
hope is that someone takes the time to do this and document what is
needed, and contribute the notes back to MS4W)
