According to this stack dump, MapServer has encountered and error and is 
trying to construct and return an ERROR URL using the "WEB.ERROR" 
directive. The crash happens when it's trying to read the HTTP_HOST 
environment variable (maptemplate.c:2384)... possibly that variable is 
not set (and MapServer doesn't check for NULL pointers in this case 
which is bad, I just filed ticket 2589 about that).

A simple fix might be to remove the WEB ERROR directive in your mapfile, 
letting MapServer produce its default error response, and see if that helps.
