A good way of verifying that you can connect to SDE is by using the 
shp2img.exe commandline utility. 
(http://mapserver.gis.umn.edu/docs/reference/utilityreference/shp2img)

Also, note that the MS4W mailing list exists for these types of 
configuration questions (subscribe at 
http://lists.maptools.org/mailman/listinfo/ms4w-users)  Maybe another 
user on that mailing list has hit your same issue.
