[OSGeo Oceania] A communications request form

Hello All,

As requested at the board meeting, below is a link to a communications
request form. The purpose is so that people with an event or a message to
convey to the community can request that the Comms Committee release the
message over the various OSGeo channels. It's a work in progress, but we
might as well try it out.
