[osgeo4w-dev] Installer update

Folks,

I have uploaded a new installer with QGIS in the express menu, and with
GRASS removed from the express menu (since it is still not really ready
for prime time as I understand it).

I have also fixed up the docs describing how to use %OSGEO4W_STARTMENU%
to create stuff in the start menu.  I see Jurgen did it properly for QGIS
even though the docs were wrong!

Let me know if anyone encounters any problems.
