[Live-demo] updated list of the content for the booklet of 4.5

Hello,

I would work on the booklet, dvd cover and desktop background if no one
else volunteers.

My questions are:

1. Is there an updated list of the dvd content - software, version? (I
have the content from version 4.0 FOSS4G Barcelona)

2. Is the contributors list in the svn already up to date? [1]
