 I ended up copying the code in both the _upgrade.sql scripts AND the
 postgis_extensions_upgrade() function. The latter is really ONLY useful to
 give users some feedback, because there seem to be NO WAY to get any
 message out from ALTER EXTENSION UPGRADE (extension-less users are
 receiving all messages)
