 upgrade: clarify incomplete deprecated cleanup errors

 Report a PostGIS-specific diagnostic when a repeated upgrade cannot rename
 a replaced function because a deprecated alias from an earlier partial
 cleanup still exists.

 Add a focused create_upgrade generator check for that replaced-function
 error path.
