I'm not sure what the best solution to this is. I applied the
krovak patch after years of "encouragement". I wasn't aware there was
a distinct approach in libproj4.
Perhaps the Krovak user community could comment on how they would like
this handled? I've no objection to using proj=kocc for this.

This is still my plan though my previous efforts to set a deadline
for myself have clearly not resulted in any progress.

It is generally my policy to advise folks who want new projections or
changes to existing projects to you, and then I try to backport the
changes as needed.
The truth is I am only spending a tiny amount of time on PROJ.4, mostly
apply provided patches, and updating EPSG database translations.
