Just a guess ..
With every new run LaTeX does not clean out old .toc files (and probably
others too)
So from time to time you should clean out those files by hand and do a
clean run from start.
Some Editors have "cleaning" in their menu and you could try that..

Most of the time the links were correct again for me after cleaning out
old files.

Probably that helps for you too
