[Qgis-community-team] update plugin structure in manual

Hi,

I did more changes to the manual structure. I removed the 2 sections (files):

a) external_plugins.tex
b) other_core_plugins.tex. 

and added a) to plugins.tex and b) to core_plugins.tex. In the core_plugins
table I added a reference column for all plugins.

Bogus, the small section external_plugins.tex was your part in the wiki. I
put your name now to the raster section. Maybe you can help Micha, who also
takes care off the grass_integration.tex?
