Did you already have a look at QGIS?
There is the coordinate capture plugin that allows to create features from GPS
positions (points and lines) - that might be a starting point.
And it's possible to create custom forms to ease entering and editing of
attributes.
Nathan's qmap[1] plugin also might have similar goals.
