Hmm I don’t see a way to do it with the google forms platform. We would also have the issue of how to translate the results back to english.

What I suggest is that you take the form, copy it and then translate it, then we can share the link. Anybody willing to do that please contact me offlist and I will share a copy of the form with you to work on, and then we can share the link to the translated form here.
