Short answer is: formal schema specs don't exist.

Long answer: the only way to determine the XML structure is via the
QGIS source code, but even then it's spread **everywhere** throughout
QGIS. And it changes version by version without any notice, with no
guarantee of backward compatibility. The official project advice is
not to try to read/write the XML directly, but to instead use the QGIS
API to handle this.
