 I have see this under ubuntu linux and qgis trunk, not under windows xp.

 Steps to reproduce:

 *) create a mapset with a projected crs (I tested with epsg 3003 and 3763)

 *) in the qgis project properties choose the (same) crs and enable on the
 fly reprojection

 *) add GRASS layers, at least one of them raster (with just vectors no
 problem)

 *) save the project and close qgis

 *) open qgis and try open the saved project, qgis will crash with

 {{{

 Error:
         GDAL Error 6: No translation an empty SRS to PROJ.4 format is
 known.
 Segmentation fault

 }}}
