I am facing the same problem since 2.12 (including 2.16). Standalone Installer, Win7 64 bit.
I did the adjustments in the processing options, no improvement.
Anyone got any idea?

Did you installed QGIS using OSGeo4w or standalone?

In the processing option, providers, make sure GRASS 7 path is pointing to
the correct folder. And disable GRASS (6).

I seen some installations where grass 7 was pointing to an old grass
installation
