I suspect you are running into GDAL bug #1736:

   http://trac.osgeo.org/gdal/ticket/1736

In particular this affects applications trying to produce GeoTIFF output
with GDAL in situations with modest sized stacks (ie. JVMs).

If this is the issue, you can upgrade to a 1.4 branch or "svn trunk" snapshot
of GDAL to correct the issue.
