Pay particular attention to the validation block... You *must* set that up or no substitution will be done.

I recently created a small test package for variable substitution, that 
contains a small working mapfile: 
http://labs.gatewaygeomatics.com/dl/ogr-outputformat.zip You could check 
that mapfile to see the syntax.  Also, a full discussion is in the 
related ticket: http://trac.osgeo.org/mapserver/ticket/3751
