[Geomoose-users] Search By Layout - Help Needed

To my prior post I was able to figure out how to tweak the search output and was able to modify so I can search on Address Number, Street Name, and Name.

However, I'd like to tweak the search form that appears on the control panel after selecting the Search button on the 2.0.1 Demo.

The top is what the demo looks like and the bottom is what I'd like my search form to look like.

How and where do I make the changes to accomplish this?
