Rich, Malcom, et al,

I have reported the same behavior for Beta 11 on Mac OS X.

What I did was

    mv dev ../..
    mkdir dev
    chmod 777 ../../dev
    ../../dev  /usr/local/grass5

That should create the fifios for you.
