sorry, my fault, it should be

g.extension.py extension=r.stream.extract operation=add                         
Downloading precompiled GRASS Addons <r.stream.extract>...
Updating metadata file...
Installation of <r.stream.extract> successfully finished
(Thu Aug 15 09:41:24 2013) Command finished (4 sec)   

or in the wx-gui: settings -> Addons extension ->  Install extension from
addons.
