Terraservice.Net DOQ data

Greetings list...

I can never get anything but a blank image when trying
to incorporate terraservice.net DOQ imagery via WMS.
Could some kind soul look at my map file and give me
some hints?  Or, better yet, could someone provide an
example of a working map file?
