James,  

I can't thank you enough.  Exactly as you said, I was not passing the LAYERS
parameter
from my initialization html file, and then it took me a while to come around
to the fact that
I was not storing the layer parameter in the template html file.

So, for any future readers of tile4ms documentation
(http://mapserver.org/utilities/tile4ms.html#tile4ms), my steps to creating
a working map from that page follow.

Initialization file - Changes marked by *****************

‘LAYERS=all’ will automatically turn on all layers.

                 
						2. name="map"	[filename] - Path, relative (or absolute) to the 
                                                                       CGI
directory, of the map file to be used.
