In the mean time, I found the an old pdf (version 1.3.2) on the server 
and modified the webpage with an appropriate URL.

http://postgis.refractions.net/docs/postgis.pdf

which aliases to

http://postgis.refractions.net/documentation/manual-1.3/postgis.pdf
