Guess you should respond to the thread on the postgis newsgroup, since
it originally came from there. I accidentally found the postgis link on
ESRI when searching for the pdf.

In the mean time, I found the an old pdf (version 1.3.2) on the server
and modified the webpage with an appropriate URL.
