Mark,

I don't have the PDFLatex part or docbook setup.  So whenever I make changes
I at most just make sure the xml passes my validation checks and that I can
build the comments files okay.

It actually passes thru everything (the building of the html etc) except the
PDFLatex part it gets hung.  So maybe someone who can build on PDFLatex can
check by changing a refentry tag to a sect1 tag.

In thinking about it, I think it breaks when I try to mix refentry and sect2
tags in the same chapter because for refentries there is usually a page
break and for sect2 there isn't (well in the html).  In the PDF -- they look
about the same to me, but I'm guessing it's the logic we have to do that
that makes the combination lethal.  Its never a problem in other chapters.

As a matter of interest, does it build locally for you when you make these
changes? If it does, then it sounds like a bug in the particular versions of
the tools on the build box.
