[QGIS-Developer] QgsAttributeDialog in standalone app

Hi all,

I am working on a standalone pyqgis application, and I'm completely stuck on the attribute dialog when adding a feature to a layer.

The dialog opens but shows a red error message 'failed to create widget with type ...' (see image attachment).
Any clue ?
